We are pleased to share that the Polish Community Council of Victoria and Polish Community Care Services PolCare have been featured in the ELDAC Linkages Program Case Stories publication. The ELDAC Linkages program aims to strengthen collaboration between aged care, primary care and specialist palliative care services, helping ensure older Australians have access to high-quality, person-centred end-of-life care.
As part of this project, we had the opportunity to share our experience and highlight the story of one of our former Home Care Package clients, Mrs Maria, who spent the final stage of her life in palliative care. We are proud to have contributed to this important initiative, which showcases practical examples of improving end-of-life care for people from diverse cultural and linguistic backgrounds.
