Government programs

The Support at Home Program helps older community members remain in their homes and live independently for as long as possible. Services are tailored to each client’s individual needs.
For people aged 65 and over needing support at home who want to maintain independence, safety, and personalised care tailored to individual needs.
PolCare provides professional in-home support services, helping seniors maintain independence, wellbeing, safety, and quality of life through personalised care.

The program provides a range of entry-level aged care services for people aged over 65 who need assistance to continue living independently at home and in their community.
For people aged 65 and over who need support to live independently at home and in their community.
PolCare provides culturally appropriate aged care support services, helping clients live independently and safely at home.


The program provides people aged under 65 with permanent and significant disability with funding for supports and services in their communities.
For people under 65 with a permanent and significant disability who require support to live independently in the community.
PolCare delivers NDIS-funded supports to eligible participants, providing culturally appropriate assistance that promotes independence, community participation, and achievement of individual goals.

The Aged Care Volunteer Visitors Scheme supports regular visits to give friendship to older people feeling socially isolated.
To be eligible, an older person must be receiving government-funded residential aged care or a Home Care Package. This includes care recipients approved or on the National Priority System and excludes those with a Commonwealth Home Support Programme (CHSP).
PolCare coordinates the Aged Care Volunteer Visitors Scheme that supports regular in-person volunteer visits to give friendship and companionship to older people who: don’t have regular contact with friends or relatives; are feeling isolated from their culture or heritage. Anyone can refer an older person to be matched with a volunteer visitor, including their aged care service provider, family and friends, or they can refer themselves. If you know someone who could benefit from companionship, refer them now.

The group provides respite, support, and tailored services to carers in the Polish community, recognising their vital role and helping them balance caring responsibilities with personal wellbeing.
For carers within the Polish community who provide ongoing support to people in need of care and require respite, assistance, and access to a range of support services.
PolCare delivers support, education, respite opportunities, and practical assistance to carers through information, guidance, and skills development, subject to available places and funding.

Our respite house, “Rose House" offers short- and long-term stays for those who wish to rest and enjoy the natural beauty of the Mornington Peninsula.

PolCare provides culturally appropriate respite care, offering short-term accommodation and support to carers and people who need a break while maintaining wellbeing and independence.
