The Care Services Manager (Care Partner) is responsible for overseeing high-quality aged care and community support services across Melbourne’s north-western suburbs. The role provides leadership in service delivery, compliance, client care coordination, workforce management, and continuous improvement in line with current aged care reforms, Aged Care Quality Standards, and Support at Home requirements. The position also supports older people to maintain independence, wellbeing and dignity within their homes and communities.

POSITION SUMMARY

The Care Services Manager (Care Partner) is responsible for the leadership, coordination, quality assurance, and continuous improvement of community aged care services across Melbourne’s north-western suburbs. The role oversees care operations across aged care and home care programs. The position ensures services are delivered in compliance with current aged care legislation, quality standards, safeguarding requirements, and organisational policies.

The Care Services Manager (Care Partner) will lead multidisciplinary teams, supervise coordinators and care staff, support workforce capability development, oversee client care planning and risk management processes, and maintain strong relationships with clients, carers, government bodies, and community stakeholders.

KEY RESPONSIBILITIES

  • Lead and oversee the delivery of high-quality, culturally responsive home care services in accordance with the Aged Care Act, Aged Care Quality Standards, Support at Home, CHSP, Work Health and Safety, privacy and other relevant legislation and guidelines.
  • Provide operational leadership, supervision and support to coordinators, care staff, administration staff and volunteers, while promoting a positive culture of dignity, respect, inclusion and continuous improvement.
  • Oversee client intake, assessment, onboarding, care planning and review processes to ensure services meet clients’ cultural, physical, emotional and social needs and support client choice, independence and wellbeing.
  • Monitor compliance, quality assurance, incident management, complaints handling, safeguarding, risk management and continuous improvement activities to ensure safe and compliant service delivery.
  • Ensure client records, assessments, care plans, progress notes and reporting systems are accurate, up to date and compliant with organisational and funding requirements.
  • Support workforce recruitment, onboarding, training, professional development and performance management, ensuring all mandatory compliance requirements are maintained.
  • Maintain effective relationships with clients, carers, allied health professionals, medical practitioners, government departments and community organisations to support coordinated care outcomes.
  • Assist with operational reporting, budgeting, resource allocation, funding compliance and organisational planning in collaboration with management and finance staff.

SELECTION CRITERIA

Essential

  • Experience in aged care, home care, community services, or disability services management.
  • Demonstrated understanding of Australian aged care reforms and Aged Care Quality Standards.
  • Experience managing client services and multidisciplinary teams.
  • Strong knowledge of home care programs, including CHSP and Support at Home.
  • Excellent leadership, organisational, and communication skills.
  • Experience with compliance, quality systems, and incident management.
  • Ability to work effectively with culturally and linguistically diverse communities.
  • Current Victorian Driver Licence.
  • Current National Police Check and relevant screening clearances.

Desirable

  • Tertiary qualification in Community Services, Aged Care, Nursing, Health Services Management, Social Work, or a related field.
  • Experience working within culturally specific or multicultural community services.
  • Knowledge of the Polish community and Polish language skills highly regarded.
  • Experience using client management systems and care rostering platforms.
